Terms & Automatic Renewal
Membership term: 12-month commitment (“Initial Term”). Memberships automatically renew month-to-month thereafter until canceled with at least 30 days’ written notice to [email protected]. Charges will occur on the 1st day of each month.
Cancellation and Early Termination
Members may cancel after completing the 12-month term by emailing [email protected] with 30 days’ notice. If cancelled early, RewindMD will recalculate any services received at standard pricing and bill the difference. No termination fee applies.
Clinic Termination Rights
RewindMD may terminate membership for non-payment, unsafe conduct, or misuse of services. Remaining Banked Funds stay as non-refundable credit.
Use of Banked Funds
Funds can be used for eligible services/products. Not redeemable for cash, transferable, or combinable with other promotions. Discounts cannot be added to promos.
Payment Authorization
Member authorizes automatic monthly billing. If payment fails, a $35 fee applies. Benefits paused until resolved.
Legal & Compliance
This membership follows California Automatic Renewal Law (ARL). Confirmation will be emailed. This is not insurance. All procedures require provider approval.
